TICKET PURCHASES
WARNING:
There are significant elements of risk in any adventure, sports, or activity associated with the outdoors, the presence of use of a motorized watercraft and activities on, in and around the water. Certain risks cannot be eliminated without destroying the unique charter activity. There will be $150.00 fine and termination of tour for anyone jumping from the boat
EXPRESS ASSUMPTION OF RISK AND RESPONSIBILITY:
I assume the risk(s) of personal injury, accidents and/or illness, including but not limited to sprains, torn muscles and /or ligaments, fractures, eye damage, cuts, wounds, scrapes, abrasions and/or contusions, dehydration, drowning, exposure, head, neck, and/or spine injuries, bites, stings, and/or allergic reactions, shock, paralysis or death. I/we elect to participate in spite of the risk.
RELEASE:
In consideration of services or property provided, I myself and any minor children for which I am responsible, any heirs, personal representatives or assigns agree that: Costa Cruises Inc. & Shaka Waka and its principals, directors, officers, agents and employees shall have NO liability of any nature for any and all damages to me and other persons or property as a result of any act, omission or negligence of Costa Cruises Inc. & Shaka Waka or any other person or entity and I hereby release and discharge Costa Cruises Inc. & ShakaWaka and insurer if any for any such damage. I also understand Costa Cruises Inc. & Shaka Waka are not to be held liable for underage drinking. I confirm that I am 21 or above in age if I am going to consume alcohol. I also confirm that if I’m under 21, I will not consume alcohol. I also confirm that I’m not using or possess any illegal drugs or prescription that have not been directed by my doctor.

Swim at your own risk. I understand that there may be filming or photography onboard for promotional purposes, signing this agrees that these photos may be used for these purposes. Personal names will not be included or used with published photos.
